By completing this registration you confirm your permission and authorize us to provide your child's name to the Jamestown High School counseling office as registered.
Your child will attend Released Time classes, which are scheduled to take place once a week during the 4th or 8th block. These classes will be recorded as an excused absence and will not replace any required credit courses. Your child will be responsible for completing any missed school assignments, and you should work with JHS counselors to select the most suitable courses and block for participation.
This consent remains valid until your child is withdrawn from the program or changes schools. School Ministries 757 reserves the right to remove any student from the program for disciplinary reasons.
Photos or videos may be taken during class or events. These images may be used in publications, promotional materials, and other media. If you do not want your child to be included in these images, please notify us in writing.
